چکیده

UNLABELLED In this case we herein report a dangerous complication from primary percutaneous coronary intervention, where an unnoticed loop of the guidewire was inadvertently made around the stent during provisional stenting. Since the guidewire and the stent were entangled, efforts to retrieve the guidewire only exacerbated the problem by compressing the stent like an accordion. We review those factors that may have influenced stent compression in our case, as well as possible ways to avoid it from occurring in the future. KEY WORDS Catheterization; Coronary stenosis; Embolism; Myocardial infarction; Percutaneous coronary intervention; Stents.

Longitudinal Stent Deformation Caused by Retraction of the Looped Main Branch Guidewire.

A 66-year-old male was treated percutaneously for a bifurcation lesion of the left anterior descending coronary artery by provisional stenting using the jailed wire technique. After successfully stenting the main branch, retraction of the looped main branch guidewire was impossible. After using an intravascular ultrasound we discovered the guidewire was entangled with a stent strut. Thereafter,...

A Novel Push-Fold Method for Removing Side Branch-Jailed Stent Struts Under 3D Optical Coherence Tomography Guidance.

A 70-year-old woman with stenosis at the bifurcation of the left anterior descending artery (LAD) and diagonal branch (Dx) underwent implantation of a Xience Alpine stent (2.5 23 mm; Abbott Vascular, Abbott Park, Illinois) in the LAD under 3-dimensional (3D) optical coherence tomography (OCT) (St. Jude Medical, St. Paul, Minnesota) guidance. 3D-OCT image reconstruction using Intage Realia softw...

Bailout technique to rescue the abruptly occluded side branch with collapsed true lumen after main vessel stenting

Guidewire recrossing into the abruptly occluded side branch (SB) after main vessel (MV) stenting in the coronary bifurcation is difficult, particularly if the SB has a dissection because the true lumen of SB is collapsed by a hematoma and the second guidewire easily goes into the false lumen. This paper reports a bailout technique to rescue the occluded SB that was complicated by a hematoma bec...

A novel approach to esophageal stent removal in the setting of proximal stenosis and failure of the primary retrieval mechanism.

A 75-year-old woman was admitted for the extraction of an esophageal fully covered metal stent (upper esophageal sphincter stent [80×20mm]; LeufenMedical, Berlin, Germany). The stent had been implanted 5 weeks earlier to treat an esophageal perforation sustained during bougienage of a benign proximal esophageal stenosis. The location of the stent was distal to the stricture, so that only a tran...

Endoscopic retrieval of a proximally migrated biliary plastic stent using a guidewire loop technique.

Endoscopic biliary stent placement is a well-established technique for various indications including biliary drainage. However, it has been shown that approximately 5% of plastic stents placed in the biliary tract migrate proximally [1]. Endoscopic retrieval of proximally migrated biliary stents is technically challenging and occasionally unsuccessful. Several techniques for retrieving proximal...
